Our team of experienced and passionate Support Coordinators work collaboratively with participants, families, and providers to ensure each person receives the right support tailored to their unique goals, needs, and aspirations. We focus on building strong relationships, promoting choice and control, and guiding participants to confidently navigate their NDIS journey.

Chelsea Cooke
Lead Support Coordinator
Chelsea is the Lead Support Coordinator at Collective Pathways and is known for her dedication, and unwavering advocacy for the people she supports. With close to 10 years of experience in the disability sector, she brings extensive knowledge, confidence, and a proactive approach to coordinating complex supports and achieving meaningful outcomes. Chelsea is passionate about empowering participants to navigate the NDIS with confidence, building strong relationships, and ensuring every individual feels heard, valued, and supported.
Maria Stefanovska
Support Coordinator
Maria is a warm, compassionate, and dedicated Support Coordinator who is passionate about helping people feel confident, supported, and empowered throughout their NDIS journey. With over nine years of coaching experience and over two and a half years as part of the Collective Pathways team, she takes a holistic, person-centred approach to building meaningful relationships, advocating for participants, and connecting them with the right supports to achieve their goals and live fulfilling lives.
Jess Watson
Support Coordinator
Jess believes that every child and family deserves to feel heard, supported, and empowered. As the parent of a child on the spectrum along with her professional background, she understands both the joys and the challenges of navigating the NDIS and the disability sector. With a warm, caring nature and a genuine passion for advocacy, Jess works alongside participants and their support networks to navigate, build meaningful connections, and create opportunities for children, young people and adults to reach their full potential.
Amy Gray
Managing Director/Support Coordinator
Amy brings a unique combination of professional and lived experience to the disability and mental health sector. Since the age of 18, she has worked across physical health, mental health, and addiction rehabilitation, while also being the primary carer for a family member and raising two children on the autism spectrum. These experiences have fuelled her passion for advocacy and her commitment to helping individuals and families access the supports they need to thrive.
Paul
Administration
Paul, alongside ensures your information is organised, complete, and fully compliant, keeping everything running smoothly behind the scenes. You may receive documents from him to sign from time to time, and he’s always there to make sure everything is completed on time and handled with confidentiality and care. Working closely with our Support Coordinators, he supports our team with essential file management so you can get the most out of your plan while your Support Coordinator stays focused on the big things that matter most to you.
